Specialized Dementia Care Program in Boarding Homes
Program Eligibility Requirements
The following information is for geared for boarding home providers.
To meet eligibility for the Specialty Dementia Care Program, the person must:
- Have a diagnosis of irreversible dementia such as Alzheimer’s disease, multi-infarct or vascular dementia, Pick’s disease, alcohol-related dementia.
- Be eligible for COPES or a Medically Needy Waiver.
- Has been assessed in the CARE system.
- Meet the additional functional eligibility criteria outlined below.
Functional Eligibility Criteria
The person with dementia must have evidence of short-term memory loss and meet at least one criterion for each section below.
The person with dementia:
- Is not oriented to place or time.
- Has limited ability to make him or herself understood through speech, writing, sign language or any other method the individual uses to communicate.
- Requires cueing throughout the task or hands-on assistance with eating or drinking.
The person with dementia:
- Ability to make decisions about daily life is poor.
- Requires reminders, cues and supervision in tasks of daily living.
- Has exit seeking behaviors.
- Is resistive to care.
- Is agitated or wanders at night.
- Has left home and become lost when trying to return.
- Seeks or demands constant attention/reassurance.
