How to Become a Community Educator for Nurse Delegation for Nursing Assistants
To apply to become an educator for one or both of these courses:

- Download and fill out a training administrator application. A training administrator provides the caregiver training classes. This involves hiring instructors, arranging for places to hold the classes, registration, reporting training data to ADSA and other administrative tasks. If you are going to be the training administrator and the instructor, please complete both applications.
- Training Administrator Application (PDF file). Please update to new files
- Training Administrator Application (Word Format). Please update to new files
- Download, review, sign, and send in a Memorandum of Understanding (M.O.U.). An M.O.U. is a document that details all of the training and administrative functions you agree to provide if you are approved to offer training.
- Send completed Instructor Application(s), Training Administer Applications, and a signed M.O.U. to:

Review process
Once the instructor application and M.O.U. have been received, it is reviewed for completeness and to ensure the instructor(s) meets the minimum instructor qualifications (PDF file) and the additional requirements listed above.
If you are approved to offer training
If your application is approved, an approval letter will be sent to you with the current curriculum, written test, training certificate, required reporting forms, and directions for how to order student workbooks, DVDs, or VHS tapes.
If you are not approved to offer training
If your application is not approved, you will be contacted directly to review or clarify any questions the reviewer may have, or receive a disapproval letter with this information.
