Updating Your Training Status Information - AFH
Once you are approved to offer training at your facility, you must notify DSHS in writing if any information from your original AFH Facility Based Training Form changes (e.g. adding a new instructor, classes, new contact information).
Use the Facility Based Training Update Form to update this information.
