Notify DSHS of Your Intent to Train - AFH
You must notify DSHS/ADSA in writing if you want to provide any or all DSHS required training (Word Format) (except Caregiver Orientation or continuing education). Notification is done by completing and mailing in an:
- Adult Family Home Facility Based Training Form (Word Format).
- Adult Family Home Facility Based Training Notification Form (PDF Format).
AFHs do not need to notify DSHS or get approval to train their caregivers in caregiver dementia, mental health, or developmental disabilities specialty trainings.
Once your AFH Facility Based Training Form is received, it will be reviewed for accuracy and staff will verify that no licensing compliance issues exist that would prohibit your facility from doing training. Learn more about when DSHS can prohibit an AFH from training.
If there is a problem, a letter will be sent to your facility explaining any concerns that must be addressed before training can begin.
If you are approved to offer training, an approval letter is sent to your facility notifying you to proceed with training. You may not begin training until you receive this letter of approval from DSHS.
If you are using a DSHS curriculum, a CD with the current curriculum, current competency tests, and training certificate will be sent with the approval letter. If you are using an approved alternate curriculum, the current competency tests and training certificate will be sent with the approval letter.
