Community Choice Guide
The Community Choice Guide is a contracted option for intensive one on one relocation support for individuals moving from qualified institutional settings to qualified community settings. These services are provided only as authorized by case management staff in the participant’s Service Plan. Services may include the following:
- Conducting an RCL Pre-Transition Consultation with potential participants. This does not include conducting the CARE assessment; or performing NF transition screen.
- Liaison acting on behalf of HCS and AAA staff between nursing facility and in-home case management, staff at nursing facility and institutional settings, housing, medical personnel, legal representatives, formal caregivers, family members, informal supports and any other involved party
- Provision of assistance and support to ensure the participant’s successful transition from the institutional setting as authorized by HCS and/or AAA staff.
Community Choice Housing Specialist
The Community Choice Housing Specialist is a contracted option for intensive one on one relocation support for individuals moving from qualified institutional settings to qualified community settings needing help locating, arranging for, and maintaining housing. These services are provided only as authorized by case management staff in the participant’s Service Plan. Services may include the following:
- Locating and arranging for appropriate, accessible housing
- Working in partnership with local housing authorities and other community resources regarding housing availability, subsidies and voucher capacity.
- Setting up the participant’s new home including services such as arranging utility hook-ups and the acquisition of furnishings and household goods
- Arrangement of the participant’s physical move from the institutional setting
- Liaison between the participant and housing provider
Informal Caregiver Support Services
Informal Caregiver Support Services include training, education and consultation aimed at helping family and other members of a participant’s informal supports begin or continue to provide care for the participant. Services:
- Are provided only as authorized in the participant’s Service Plan
- Include support groups, education, and training on topics such as self care and stress management
- Include education and training regarding disease and diagnosis specific issues, including assistance with challenging behavior management
- Include brief solution focused family, group, or individual counseling
- May be provided in the institutional setting, home, or other qualified community setting
Informal Caregiver Respite
Informal Caregiver Respite is the provision of hourly services for the caregiver for the purpose of time-limited, intermittent respite from caregiver responsibilities. Services:
- Are provided only as authorized in the participant’s Service Plan
- May be provided through IP or Agency Care in the client’s home.
Challenging Behavior Services
Challenging Behavior Services include training, education, and consulting services which benefit individuals with traumatic brain injury as well as those with a diagnosis of mental illness. It targets direct interventions to decrease aggressive, destructive, sexually inappropriate, or other behaviors that compromise a participant’s ability to remain in the community.
Services:
- Are provided only as authorized in the participant’s Service Plan
- May Include training, behavior support plans, and specialized cognitive counseling
- May be provided in the institutional setting, home, or other qualified community setting
- May also be provided in common community settings that the person needs to navigate (i.e. stores, offices, parks etc.).
Life Skills Services
Life Skills Services impacts directly on participant’s ability to access community settings or health services. It also helps individuals and their caregivers with personal skill development related to the individual’s care plan. Services:
- Are provided only as authorized in the participant’s Service Plan
- Must help the consumer achieve measurable goals and objectives:
- May include training, education and consultation to help participant to access community settings, or health services.
- May include personal skill development related to Service Plan
- May include self-advocacy training
- May connect participant with less formal or atypical daily activities, such as those that might be found through local YMCA-type organizations, Senior or Community Centers, etc.
- May be provided in the institutional setting, home, or other qualified community setting
Professional Therapy Services
Professional Therapy Services covers a range of services for participants whose needs include therapeutic services not otherwise covered by Medicaid. It covers supports and services that are typically performed or provided by people with specialized skill, certification or licenses. Services:
- Are provided only as authorized in the participant’s Service Plan
- May include Physical, Occupational, Speech or other professional therapy
- may be provided only in those instances where the Medicaid benefit has been exhausted or denied
- May be provided in the institutional setting, home, or other qualified community setting
Transitional Mental Health and Substance Services
Transitional Mental Health and Substance Services include mental health and/or substance abuse services for participants whose needs are not otherwise covered by Medicaid. Services:
- Are provided only as authorized in the participant’s Service Plan
- are for participants transitioning from institutional to community settings
- May also include mental health services for instances in which the authorized level of services do not meet the participant’s transition needs.
Example: A participant who may be experiencing depression or anxiety related to their transition.
Service Animals
Service Animals are provided only as authorized in the participant’s Service Plan. Services include:
- The provision of a trained service animal
- Specific training of the service animal to meet the individualized needs of the client
- Orientation of client with service animal
- Service animal training with client
- Consultation with client as needed
