Contact your training program director and request a signed test application with the facility code or contact the WA State Nursing Assistant Registry (360) 725-2570.
If a candidate trained in WA State or another state and it's been over 2 years:
The student will need to retake the nursing assistant training and state test. You must test within 2 years from the date of your certificate of completion from the nursing assistant training.
If a candidate trained in "another state" and it's been less than 2 years:
Fax or mail Certificate of Completion / training certificate to the Nursing Assistant Registry with their mailing address, phone number and the breakdown of clinical and classroom hours. If they don't have this information on the certificate then they could request this information in a form of a letter and have instructor sign it.
If you are an RN from another country:
Please fax or mail the transcripts with grades for approval. Nursing students may be able to sit for the exam if they have successfully completed classes that qualify them to test.
If a nursing assistant certified has not worked as a caregiver in the past 24 months and desires to work in a nursing home, they will need to retrain and retest.
