PROVIDER SATISFACTION POSTCARD SURVEY

Dear Boarding Home Provider:

Residential Care Services (RCS) plans to initiate a new provider satisfaction survey effective October 1, 2003, in an effort to collect input from providers regarding their interactions with RCS staff during RCS visits to facilities.

Information from providers who answered surveys performed in 2000, 2001, and 2003 has been very useful to the department as we revise procedures, train staff, and seek the most effective methods for working with the providers, families, and residents.

Between October 1, 2003 and September 30, 2004, at the beginning of each RCS survey or reinspection visit to a facility, RCS staff will hand a postcard survey to each provider. I am hopeful that each provider will complete the postcard survey and return it, postage prepaid. Individual survey responses will be kept confidential. Survey results will be aggregated for RCS use.
