SUBJECT: CHANGE IN PROCESS FOR OBTAINING
BOARDING HOME BACKGROUND AUTHORIZATION FORMS

Dear Boarding Home Provider:

The Department of Social and Health Services (DSHS) Forms and Records Management Services provides access to many of the forms and publications used by providers through their web site at http://www.dshs.wa.gov/dshsforms. By accessing this web site, you are able to download DSHS forms (many which are available in multiple languages), order DSHS publications (brochures, booklets, posters, flyers, and pamphlets), as well as order some DSHS forms that are stocked in the warehouse.

This letter is to inform you of a change in the process for obtaining the Boarding Home Background Authorization Form, DSHS #09-828A. The DSHS Warehouse no longer stocks this form. This form is now in electronic format only and can be downloaded from the DSHS Electronic Forms web page at: http://www.dshs.wa.gov/dshsforms/forms/eforms.html Once on this page, please scroll down until you see the number for the corresponding form.

The Notice of Transfer and Discharge (DSHS #10-237) will soon be available on this website. Because the information posted on the Forms and Records website will be updated over time, we recommend checking it on an occasional basis. If you have further questions, please call DSHS Forms and Records Management Services at (360) 664-6120.

As you may already be aware, state budget cuts will effect some of the processes you have been accustomed to, including how you obtain copies of some forms vital to operation of your boarding home. If you do not have access to a computer, it will be necessary for you to identify a resource in your community, such as the local library, where you can use a computer to access this information. I appreciate your commitment to work with the department as we embrace alternative methods to continue to conduct business.
