Adult Family home provider training - Region 1
Dear Adult Family Home Provider:
Residential Care Services (RCS) and Home and Community Services (HCS) will offer Adult Family Home (AFH) provider training on the Comprehensive Assessment and Reporting Evaluation (CARE) Tool and a review of the negotiated care plan.
Your training session is scheduled as follows:
DATE: October 14, 2004HOURS: 10:00 a.m. to approximately 3:00 p.m.

LOCATION:
Sacred Heart Medical Center

- The training will be held in the Providence Auditorium.
- The training is free of charge.
- Pre-registration is not required; a sign-in sheet will be available upon arrival.
- Parking is free on the upper level lot on a first-come, first-served basis and paid parking is available on the street.
- Coffee/food will not be served and you may not eat or drink in the auditorium; however, there is a cafeteria on site.
RCS will continue to hold the regional forums throughout the year and I encourage you to attend as often as possible. These forums are an opportunity for providers, provider associations, and advocates to share their concerns and views with RCS staff and each other, and for the department to share information. Do not park in the office parking spaces, in front of the Department of Licensing, Déjà Vu, or around the building, as you would need a parking sticker.
- Coffee/food will not be served. Soft drinks and snacks are available for purchase on-site.
