SUBJECT: PROVIDER TRAINING DATES & LOCATIONS

Dear Adult Family Home Provider:

Due to budget constraints, we have had to make changes to the Adult Family Home (AFH) provider trainings scheduled for this fall. All trainings will include an overview of the new Medication and Emergency Evacuation and Safety WACs, presented by Robert Stroh of Residential Care Services. The following is a listing of training dates, times, and locations:

Date: TUESDAY, OCTOBER 22, 2002
Time: 9:00 AM to 12 noon

Date: THURSDAY, OCTOBER 24, 2002
Time: 9:00 AM to 12 noon

Date: MONDAY, OCTOBER 28, 2002
Times: Session 1: 9:00 AM to 12 Noon
Session 2: 1:00 PM to 4:00 PM

Location: SHS/DDD Training Room (seats 75)

Tacoma, WA 98402

Use parking garage adjacent to the building and inform attendant that you
are there for a meeting at DDD.

Maps to each location are enclosed. There is no registration, but there is limited seating. No continuing education credits/hours will be offered. Each session will start and finish on time.
