SUBJECT: NOMINATIONS FOR THE NEW
ADULT FAMILY HOME ADVISORY COMMITTEE
Dear Adult Family Home Provider:
During the 2002 legislative session, House Bill 2444 created a new Adult Family Home Advisory Committee established by the Department of Social and Health Services, Aging and Disability Services Administration. This Adult Family Home Advisory Committee will replace the former committee managed by the Department of Health.
Committee members selected will represent residents and/or family members, adult family home providers and associations, the Long-Term Care Ombudsman Program, and the Resident Council of Washington. The committee will be comprised of eight members, including an appointed chairperson.
Committee members should be available to attend all advisory committee meetings that will be held quarterly for approximately three hours during a weekday at our headquarters office in Lacey. Some committee activities may require additional meetings. Mileage reimbursement will be available for committee members.
We are currently accepting nominations for potential members to serve on the Advisory Committee for a two-year term, beginning in August 2002 and ending in August 2004. If you are interested in participating as a member of the new Committee, or if you would like to nominate someone else to be considered, I invite you to submit the enclosed nomination form.
Please submit your nomination using the enclosed form. All nominations must be postmarked or received by fax on or before June 15, 2002 to be considered. Residential Care Services will review all nominations, make selections, and notify candidates. Appointments will be made and all candidates notified by July 15, 2002.
