Chronic Care Management Project Evaluation Report
Aging and Disability Services Administration (ADSA) and Health and Recovery Services Administration (HRSA) have completed an evaluation of the effectiveness of the interventions provided by the Chronic Care Management Projects (CCMP) as requested by Substitute Senate Bill 5930.
The projects were designed to improve the health of enrollees by providing:
- Evidence-based assessment and interventions.
- Coordination of health care and supportive services.
- Education and training in improving self-management skills and improving functional and self-care abilities to slow progression of disease or disability.
