There are two ways to get this kit:
- Order a Caregiver Kit by phone or email. Call Aging and Disability Services Administration between 8 am and 5pm, Monday through Friday at 1-800-422-3263, or send and email to
KorteLM@dshs.wa.gov. Make sure to include your name and mailing address. - Read the materials online.
