Policies and Procedures Attestation Form (DSHS 16-197)
Dear Boarding Home Provider:
Washington Administrative Code (WAC) 388-76A-2600 requires boarding homes to develop, implement, and maintain policies and procedures designed to help staff know what to do in given situations and how to meet resident’s needs. In most cases, if you simply restate the WAC, the policy and procedure will not provide sufficient direction to your staff.
Boarding homes must:
- Develop, implement, and maintain the policies and procedures necessary to operate their facility; and
- Make sure that their policies and procedures comply with licensing and other applicable state and federal legal requirements.
If you consider including the following in your policies and procedures, they may provide useful direction for staff:
- When the staff are expected to take action(s);
- Who will be responsible for talking the action(s);
- What action(s) the responsible person should take;
- How those action(s) should be carried out; and
- Where the action(s) should occur, if appropriate.
You must:
- Complete and sign the Boarding Home Policies and Procedures Attestation form as part of the boarding home license application process.
