Centralized Criminal History Background Unit

Dear Boarding Home Provider:

In August 2000, the Department of Social and Health Services consolidated the various divisional criminal background check units into a single centralized unit. The address is Post Office Box 45036, Olympia, Washington 98504-5036.

Aging and Disability Services staff has updated the mailing address on the Background Inquiry Appliation Form DSHS 09-828(X) and has made address and phone number changes to the “Criminal Conviction History Booklet – A Resource Guide for Licensed Boarding Homes”. The revision date on the front cover of the booklet has not changed.

Also note that the two new disqualifying crimes adopted by the 1999 Legislature, First and Second Degree Custodial Sexual Misconduct, have been added to the list of automatically disqualifying crimes located on the back of the Background Inquiry Application – DSHS 09-828A(X) (REV. 10/2000).
