Dear Boarding Home Provider:

The department has held a number of trainings throughout the state to inform providers of mandatory abuse reporting requirements. Materials available at the training were revised throughout the process. I am enclosing a copy of the Final Reporting Requirements –
October 2000. Please discard the Reporting Requirements document enclosed in the
Dear Provider letter dated September 21, 2000, and the material you received at the
October trainings. This enclosed corrected copy with Attachments A & B is the document
that will be referred to by licensors and providers.
