Recently, I had the opportunity to hear from a number of you as you voiced your concerns and wishes pertaining to this publication. Residential Care Services is committed to responding to concerns that are in the best interest of Long Term Care residents.
Enclosed you will find a professionally printed copy of the booklet, “Your Legal Right to Make Decisions About Health Care and Advance Directives in Washington State, 1999”.
Effective immediately, this booklet is available to you, free of charge, by faxing the DSHS Warehouse.
- Instructions for ordering copies of this booklet are listed on the back cover. You may order up to a maximum of 50 booklets, per order. The DSHS Warehouse will not ship orders to a PO Box, so please indicate your street address when placing your order.

The following are important answers to questions commonly asked about this
publication:
- The advance directive laws in Washington have not changed; the booklet has been revised for clarification.
- You are required by law to provide Washington State information on advance directives to new admissions, regardless of payment status. (Current residents should have already received this information when admitted to your facility.)
- The revised booklet does not require replacement of any existing advance directives, nor does it question the validity of any existing advance directives.
- Additional resources including a values history form, glossary of terms, and decision making chart, can be ordered by contacting AASA at 1-800-422-3263.
