RE: Individuals with Challenging Support Issues
Dear Providers:
Aging and Disability Services Administration (AASA), the Division of Mental Health and the Regional Support Networks (RSN), and the Division of Developmental Disabilities are working to improve coordination and communication regarding DSHS clients with histories of challenging behaviors.
DSHS has developed the enclosed form (DSHS 10-234) to document challenging support issues. It contains an original and two copies (yellow and pink). Beginning December 1, 1999, case managers will complete this form for all potential residents who are DSHS/RSN clients and who have challenging support issues. Case managers will give both copies of this form to providers.
AASA is requesting that all contracted providers establish a central file where the pink copy may be filed. The pink copy does not contain the resident's identification. When a case manager needs information about current residents who may pose a risk to a potential new resident, the case manager may then review this central file. Case managers can learn of the presence of an individual with challenging support issues without violating the confidentiality of a specific resident. When the resident moves out, providers are asked to remove the corresponding pink copy from the central file.
The yellow copy of this form should be placed in the resident's record, and considered when developing the resident's plan.
