Enclosed is a print-ready copy of the revised booklet referenced above. This booklet was first developed in 1991 in response to the requirement of the Patient Self-Determination Act (PSDA). It was revised in April, 1992 due to changes in state law. This booklet has been revised to provide greater clarity and understanding of the application of existing laws and optimal approaches to these issues.
For your convenience, this booklet is abbreviated in length and can be reproduced on standard (8 ½" x 11") paper. You may duplicate the number of copies needed for residents and family members. Sample forms have been omitted. The related Values History Form, Glossary of Terms, and Decision Making Chart have been removed from the booklet itself, but are enclosed in print-ready form for use as an additional resource.

Besides the enclosed English version, the booklet will be translated into Russian and Spanish; these translations will be available at a future date.
