SUBJECT: REMODEL/ADDITION TO LICENSED BOARDING HOME
Dear Boarding Home Provider:
The notification process for a remodel license inspection is being changed. Currently providers submit a license application to notify Residential Care Services when a boarding home has been remodeled and is ready for a remodel license inspection. The licensee includes license fees when the remodel/addition will increase the capacity of the boarding home. Since the Department of Health notifies Residential Care Services when a boarding home is ready for remodel license inspection, a license application is unnecessary. The procedure outlined below streamlines the notification process:
Boarding Home Provider
- Completes construction application process through Department of Health, Construction Review Services.
- Sends Department of Health a pink card reporting construction is complete.
Residential Care Services
- Receives notification from Department of Health when the facility is ready for remodel license inspection.
- Conducts a remodel license inspection, prepares an updated Licensed Room List, and forwards Licensed Room List to Licensee.
Boarding Home Provider
- Receives Licensed Room List and identifies rooms to be licensed.
- Retains copy of Licensed Room List and forwards original to Residential Care Services.
- Licensee submits Change in Status form to Residential Care Services, Application Intake Unit, if a capacity increase, capacity decrease, or change in use of rooms is desired.
