SUBJECT: FORM FOR CHANGE IN STATUS TO LICENSED ROOMS

Dear Boarding Home Provider:

In April you were sent Dear Provider letter AASA: BH 99-004 regarding Boarding Home Interim Guideline #33: Licensed Rooms. At that time the revised form to be used for the Licensed Room List should have been enclosed, but was inadvertently omitted.

Attached to this letter is the revised form, which should be used when there is a change in the licensed resident bed capacity or in the use of resident rooms (i.e., adding, subtracting, or changing the use of licensed rooms.)
