BOARDING HOME – CHJANGE OF LICENSEE

Dear Boarding Home Provider:

Questions have arisen regarding how a change of licensee if determined for boarding homes. Enclosed is a summary sheet that defines what events constitute a change of
licensee, and instructions on notifying the department when such a change is planned.

Please note that real estate changes do not necessarily constitute a change in licensee. However, you must notify the department, in writing, at least thirty (30) days prior to selling, leasing, or renting the boarding home.
