PLAN FOR THE UNEXPECTED
Dear Adult Family Home Provider:
This is a reminder should you experience an unexpected event that results in you being unable to continue to operate your home. Preplanning will ensure residents continue to receive the proper care and services needed should an unexpected event occur.
Providers Must Remember:
- A license is required to operate the home.
- The license is only valid for:
- The person named on the license.
- The address stated on the license.
- Anyone else operating the home, including a family member, is operating an unlicensed home.
Some Unexpected Events That May Occur:
- The home must be closed as quickly as possible for some reason.
- The provider leases the home and your landlord notifies you that you must move by the end of the month.
- A family emergency causes the provider to go out-of-state or country.
- The provider’s unexpected serious illness, accident or death.
Be Prepared -- You Should Plan For Unexpected Events:
- Develop written plans and procedures for what must be done if you become unable to continue to operate the home;
- Train your staff; and
- Inform your family members of the plan.
Immediately Notify the Department:
- If an unexpected event occurs which makes you unable to continue to operate the home, call or have someone immediately call the department at 1-800-562-6078 and contact your local RCS Field Manager. This way we can all help to ensure your residents receive the necessary care and services.
