NOMINATIONS FOR ADVISORY COMMITTEE MEMBERS
Dear Adult Family Home Provider:
The Department is seeking nominations to fill positions on the Adult Family Home Advisory Committee. Current member terms will expire on December 31, 2007.
The 2002 Legislature established the Adult Family Home Advisory Committee to ensure a cooperative process among the department, adult family home provider representatives, and resident and family representatives on matters pertaining to the adult family home program. The Committee is composed of nine members representing the adult family home industry.
Individuals nominated to fill these positions should be available to attend all Committee meetings that are held quarterly for approximately three hours during business hours on weekdays at our headquarters office in Lacey. Some Committee activities may require additional meetings. Mileage reimbursement is available for Committee members.
If you are interested in participating as a member of the Committee, or would like to nominate someone else for consideration, I invite you to submit the enclosed nomination form.
Nominations must be postmarked, faxed or e-mailed on or before December 5, 2007.
