2006 AFH "Dear Provider" Letters

NOMINATIONS FOR ADVISORY COMMITTEE MEMBERS

Dear Adult Family Home Provider:

During the 2002 legislative session, an Adult Family Home Advisory Committee was established in an effort to ensure a cooperative process among the department, adult family home provider representatives, and resident and family representatives on matters pertaining to the adult family home program. The Committee is composed of eight members representing the adult family home industry.
The Department is seeking nominations to fill four two-year positions on the Committee for members whose terms are expiring. These positions must be filled as follows:

Individuals nominated to fill these positions should be available to attend all Committee meetings that will be held quarterly for approximately three hours during business hours on weekdays at our headquarters office in Lacey. Some Committee activities may require additional meetings. Mileage reimbursement is available for Committee members.

If you are interested in participating as a provider member of the Committee, or would like to nominate someone else for consideration, I invite you to submit the enclosed nomination form.

Nominations must be postmarked or received by fax on or before January 27, 2006 to be considered.   Residential Care Services will review all nominations, make a selection, and notify the candidates. The next Adult Family Home Advisory Committee meeting is on February 9th at 9:00 AM.  The meeting will be held in the Blake East Building in Lacey in the Rose Room.
