AFH ANNUAL FEE – DISCONTINUATION OF SECOND BILLING NOTICE

Dear Adult Family Home Provider:

This letter supersedes ADSA: AFH 2003-021,which was issued November 28, 2003.

The purpose of this letter is to inform you of a change that will be occurring related to the billing notices of your annual fee. Currently, the Office of Financial Recovery (OFR) sends you the first and second billing notice and coupon for payment of your annual fee.

Effective immediately, OFR will stop sending you a second billing notice and coupon. You will receive only one billing notice and coupon for payment of your annual fee. This billing notice will be mailed to you on the 15 th of the month that is 60 days prior to the month your annual fee is due. For example, if your annual fee is due October 15, 2005, your billing notice and coupon will be mailed to you on August 15, 2005. You will not receive a second billing notice and coupon.

Please make any corrections or changes to the information on the statement in the section of the invoice that instructs you to do so. It is very important to update the invoice with any needed corrections, so Residential Care Services (RCS) has accurate information about your home.
