2005 AFH "Dear Provider" Letters

NOMINATION FOR AFH ADVISORY COMMITTEE MEMBER

Dear Adult Family Home Provider:

During the 2002 legislative session, an Adult Family Home (AFH) Advisory Committee was established in an effort to ensure a cooperative process among the department, AFH provider representatives, and resident and family representatives. The Advisory Committee is composed of eight members representing the AFH industry, and the Committee addresses matters pertaining to the AFH program.

The Department of Social and Health Services, Aging and Disabilities Services Administration (ADSA), is seeking nominations to fill a vacant two-year position on the Advisory Committee. The vacant position must be filled by an AFH provider.

Individuals nominated to fill this vacancy should be available to attend all Advisory Committee meetings that will be held quarterly for approximately three hours during a weekday at ADSA headquarters in Lacey. In addition, some Committee activities may require additional meetings. Mileage reimbursement is available for Committee members.

If you are interested in participating as an AFH provider member of the Advisory Committee, or if you would like to nominate someone else to be considered, I invite you to submit the enclosed nomination form.

All nominations must be postmarked or received by fax on or before April 27, 2005 to be considered. ADSA, Residential Care Services will review all nominations, make a selection, and notify the candidate. The appointment will be made and the candidate notified by May 10, 2005. The next AFH Advisory Committee meeting is on May 28 th at 9:00 a.m. The meeting will be held in the Blake East Building in Lacey in the Rose room.
